The explanation
A pour depends on access, grade, drainage and prep, so nobody can price it from a menu. What can be self-served is the estimate visit. The form collects what you need to decide whether the job is in your range: location, project type, size and a few photos.
Asking for dimensions and photos filters out leads that are too small, too far or out of scope. Offer a choice of visit windows, and set a lead time so you are not asked for tomorrow when your week is full.
Be clear about what happens next. A confirmation that says your request is pending, and that you will confirm the visit after reviewing the photos, avoids the impression that a date is already fixed. The pour date comes later, after the quote is approved.
