Start with the fields every visit needs: name, phone, email, the address where the work happens, and any gate code, pet or parking note. Add a free-text notes area for the things you would otherwise tell the next person out loud.
Next, keep history on the record. When you finish a visit, log what was done, what was said, and what to do next time. Six months later this saves a tech from asking the customer to repeat everything, and it shows the owner which customers are overdue for a return visit.
Pick a single home for the record and make every other tool read from it. A shared spreadsheet works for a small business if everyone uses the same columns. Once more than a few people touch it, a customer database that keeps changes in order is safer, since two people editing one row is how information gets lost.
