The main scheduling pains are late cancellations, empty slots and a waitlist that nobody has time to work.

- Sessions repeat. A client booked every Tuesday at ten holds that slot, so the open calendar is whatever recurring clients have not already claimed, which shrinks fast.
- The gap between sessions matters. Clinicians need time for notes and a break, so slots are rarely back to back, and a booking tool must respect that buffer.
- Cancellations are costly because the slot cannot be resold at short notice. A clear policy with a notice window, agreed at intake, protects the clinician more than any reminder does.
- Licence rules for clinicians vary by state and by credential. Confirm with your board what you may do by phone or video across state lines before offering sessions to anyone outside your state.

Questions owners ask
How should a therapy practice take new client requests online?
Offer a short form that asks what the client is seeking and when they are free, and have the clinician review it before anything is confirmed. Many clinicians do not want an instant booking from a stranger. A request that arrives pending lets them screen for fit first.
Do recurring weekly sessions need special handling?
Yes. Create the series once so the client holds the same day and time, and make pausing and skipping simple for vacations. Review the series now and then, because a client who quietly stopped coming still occupies a slot a waiting person could use.
How do I reduce late cancellations and no-shows?
State the notice window and fee at intake and have the client accept it in writing. Send a reminder the day before, with a one-tap cancel or reschedule link so a client who cannot come tells you early instead of staying silent.
Should a counselor keep a waitlist?
Yes, if cancellations leave open slots you cannot fill by phone. Keep a short list of clients who want an earlier time, and offer a freed slot to them in order. Without a list, the empty hour simply goes unused.
What should intake collect before the first session?
Collect contact details, the reason for seeking care in broad terms, insurance information if you bill it, emergency contact and an acknowledgment of your policies. Keep clinical history in your clinical record system, not in a scheduling tool, so the two stay separate.
Can sessions be held by video?
Yes, many practices offer both. Each video session needs a meeting link sent with the reminder, and the clinician needs to know that the client is located in a state where the clinician is licensed. Check your board's current rules before offering video to out-of-state clients.
How long should a session slot be on the calendar?
Block the full session length plus a buffer for notes, even if the clinical hour is shorter. A fifty minute session with a ten minute gap is a common pattern. A calendar that shows only the fifty minutes invites back-to-back booking and a missed lunch.
Are reminder texts safe to send for therapy appointments?
Keep the text content minimal: a time and a reply instruction, with no clinical detail. Texting needs the client's consent and a registered sending number, and some clients prefer email or no reminder at all. Offer a choice at intake and honor it.
What happens when a client wants to see a different clinician?
Let the client request a clinician by name or by specialty, and have the practice owner approve changes so that caseloads stay balanced. Keep each clinician's calendar private from the others, so only the front desk and the owner see the full picture.
What should a counselor move off paper first?
Move the appointment calendar and reminders first, because missed appointments are the most expensive leak. Online intake forms come next. Keep clinical notes in a system built for clinical records, since a scheduler is not one, and keep the two apart on purpose.
